What do I need to bring with me to my appointment?

• The  completed Patient Intake Packet.
• Any diagnostic tests results such as Xrays, MRI, or CT Scan results.
• Any list of exercises that have been provided to you from other healthcare personnel.
• Wear loose fitting clothing to your appointment.
